failing MLPS and or connector to sensor issue.the original was prone for water intrusion and has been revised.if this is a recent 20k mile rebuild,it "should" have been updated at time of trans install.
the issue might not toss a code.but you don't need one.that's the first place to go to.
down on the drivers side of the trans, directly in back of the shift linkage/cable is a sensor.this is the one.carefully unclip it,and spray it (vehicle side plug, not down in the sensor) really good with electrical cleaner.apply a little dialectic grease and snap it back in.it might be as good as gold from now on.if not, just replace it with oem (oem comes with instructions) make sure to get the one that comes with the updated connector,so you can re-pin it.this would be the updated '95 mlps.if you buy the updated one,and it simply plugs in,then no re-pinning is required.it has already been done.
if this ever happens again, quickly slide the shift lever into N and slow way down or just stop before shifting back to D,so the trans doesn't bang into gear.then replace the sensor asap.
while your at it (unrelated/for maintenance) pull the plug on the pass side of the trans, and clean this one up the same way, making sure each plug fully seats/clips back in when done.
cleaning the relay pins, and the main ecm harness the same way doesn't hurt either.

You can pull the codes with a paper clip, no code reader needed. See Steve83s Supermotor site for info and a list of the codes.
