It could be a Bad SGII Code
Does the Sync code your using now worked in the Past??? If so maybe theres a wire chaffe somewhere
If it didnt Acheive sync it wouldnt start Period untill it did sync

at idle ipr 20.x-22.x icp 635-645 temps ect and eot 190-192
Just towed the trailer today 40 foot gooseneck combined Truck and trailer wt 24.5K
Crusing 60mph i seen ect as high as 205 eot 213-215 at that coolant temp
The fan never runs faster than low 500's Rpms towing Outside temp low 60's At idle is 325-375 At 190 temps

Hpo system looks good no Leaks
With that said makes me wonder if this is an electrical problem being that its intermitened
You dont have the Hard/Long start everytime do you???
Your Fan seems ok per those temps but wouldnt hurt to check the Fan Clutch wireing at the fan shroud as a wire chaffe there will cause a NO START
Still Have the EGR Valve??
