Mine did the same thing until it finally stopped starting altogether, but up until then it idled very rough until I gunned it a few times. Found out I had a bad neutral safety switch AND a bad lead on one of the connectors to the left bank (it only takes one of those little male OR female connectors to screw you up inside the main connectors). Once the two items were fixed, it starts and idles just fine.
I think it might be the DMF (I believe it is the original, could be wrong, I'm the 3rd owner, 160K) and it has been giving some symptoms of age lately also. Gonna go pull the inspection cover now.
What did you find when you pulled the inspection cover? The dmf needs to be eliminated as a possible cause of your problems. I just reread the posts on this thread and didn't see the results of removing the cover and testing for any looseness in dmf and clutch. I could have missed it though.
