The answer

Warped pump... Transmission rebuild... Took the transmission guys two rebuilds to get it right. They tried to be cheap and not replace the pump.

That P0728 is an intermittent speed sensor, and the P0768 is a bad shift solenoid. My PC/ED didn't have the codes, but my AE did.
