2003 Navigator plug ejects

2003 Navigator plug ejects
72k miles. Plug ejected by it's self. What causes this? Any signs of this happening before hand? Looking like a major repair, This is my brother in laws truck. Also, any thing you can do to help prevent this?

Improper torque is the main answer you will receive/hear. Were the plugs recently changed?
However, you don't hear about plug ejections on Navigators much. Mainly its the 97 to 03 Expeditions.
