F250 rear brake adjusters - $%#@&

I went looking for some instructions on which way to turn the rear drum brake adjusters on my '78 F250 and it seemed the consensus was that you put the tool in at the bottom of the slot and rotate the adjuster upwards. In other words, the top of the adjuster rotates away from the centerline of the truck.

I did this until the adjuster ran out of travel but the brakes never got tighter, in fact they got looser just based on the travel of the parking brake pedal. Now the adjuster is stuck all the way in the relaxed position and I'm either going to enlarge the hole in the back plate to get a grip on it with some needle nose vice grips, or if that fails pull the drum and go at it from the front.

So did I get some bad info on which way to turn them, or is it possible that that adjuster was installed on the wrong side of the vehicle by accident?

Self adjusting brakes

To tighten self adjusting brakes the brake spoon is pushed down.

According to this thread I was doing it correctly by pulling down on the handle of the tool, which rotates the edge of the adjuster upward as you're looking through the hole. And doing that loosened the adjuster. So I ask again, are my adjusters installed on the wrong side? (Or at least the one on the side I'm currently working on?)
