I have been told that it requires 2 previously programed keys to program a third with the instructions found in the owners manual. Otherwise the dealer has to erase and reprogram at least 2 new keys using special equipment.
My wife drives the 1997 Expy we have. We needed to make another key for it shortly after we bought it (used). The procedure was spelled out in the manual. I can't reference it now as my wife has the truck. As memory serves, it is something like this:
insert original key
turn key to on (do not start)
turn key to off
remove original key
insert new spare key
turn to on (don't start)
theft light should flash, then go out (I think it will do this)
turn spare key to off
remove.
done.
Look in your manual for something like this. The dealer told my wife that he didn't think I could not do this. Go ahead and try but I think you will be back. I told her no way. I know I read it in the manual that I can program the key (not the key fob). So I did it.
If I think about it I will look it up and repost the directions.
I do know this - you can only attempt to program keys a set number of times - like 25 or fewer. Check it out.

Big's got it right. For the '97 and '98 models, you only need one original key to program another. for the '99 to current models, you need 2 original keys to program a third. Also, for the '97 and '98 models, you can freshly program a key if you have no other key for it, but it's a timely procedure.
