I had mine replaced by Safelite on 10/20/09. I requested the Super Duty logo and they offered the upper sun tint as well. It was covered under my comprehensive auto insurance with a $100.00 deductible.
Parts: $362.78
Labor: $133.00
Sub Total: 495.78
Sales Tax: $29.93
Total: 525.71
BTW, the windshield had a 1 inch size crack approximately 6 inches below the upper edge on the driver's side. Safelite attempted to "repair" the window but ended up extending the crack to approximately 3 inches. The technician stated that the vacuum caused the crack to expand. It was at that point that I requested that the window be replaced.

Reading this post, I am for once glad I am in MA. Every level of insurance comes with 100% glass coverage. And this means all glass, not just the windshield. Side glass, back glass, moonroof, and if your headlights are housed in glass, they are covered too.
