Pulled Codes - Now what?
Something I have seen is that just because the spout plug is present doesn't necessarally mean it's doing the job and completeing the circuit, if one of the tabs is broken it won't make contact, so pull it out and measure it with a DMM, it should be a direct short or 0 ohms. If it measures anything else replace it.

Also, shouldn't I have vacuum to the EGR with the engine warmed up and idling? I pulled the line off and there is none. All other vac lines are hooked up and EGR solenoid is plugged in.

No you should not. You only have vacuum at mid throttle with a warm engine. No vacuum at idle or WOT.
I checked out the wiring for spout and coil. It all seems fine.
I also played with the timing and got shocked when I bumped into one of the spark plug wires, must be time for new wires.
The truck also has a new condition- idling or cruising at steady throttle it will start to miss and if I give it more throttle it clears up and runs fine.
I pulled codes and got the same as I did the first time.
