There are 2 rear fender bolts...one upper accessed thru the hole or by removing the small metal plate held on with one screw...depending if your truck has them or not...I recently pulled a fender where this nutplate died...big surprise...whet I ended up doing is slowly 'feeling' as I slowly spun the nut/bolt to where it got a bite. With the bite held I got a large flathead/str8slot screwdriver and put it parallel where the bolt is approxamately between the cab itself and the fender and put more tension on the fasteners by prying outward and slowly undoing the bolt from the nutplate...it will prolly break free and will need to reset the bite and prying as you go...but it will work...this hole in the cab itself is slotted slightly so the flat part of the nut plate will bite...may need to try to **** the bolt at an angle as its removed to get a better bite. Use a 1/2" drive ratchet and go slow...its a pain, but easier then drilling/chiseling as its a high spot inside the cab near the dash and E Brake handle.
The 2nd location is the lower bolt you found already...this setup is identical to the upper cab bolt I described above...this one would be a good one to drill/chisel/grinder/cutoff wheel if you have the tools, etc.
