How long have you had the truck? How many miles has the oil been in the engine (and is it still full)? Did the problems start right after you switched to synthetic? We have one member here who's truck didn't like the synthetic (kwikkordead). At what interval has the oil been changed in the truck? To check for a stuck open thermostat, you'll have to feel the radiator hose to see if it seems to be warming up with the engine instead of staying closed and keeping the coolant in the block.
If you talk to any more shops, ask them how they came up with their diagnosis. Is it from their experience (aka guessing) or did they hook it up to a computer for diagnostics? If they used a computer, what readings did they monitor to come up with their proposed repair?
As Rick mentioned, monitoring ICP psi, IPR %, EOT, to get a gauge on things. A buzz test on a cold engine, followed by repeated buzz tests, will tell you if there might be some injector issues with the cold oil. A good shop can also do a contribution test on the cold engine by wiring in a resistor on the EOT sensor.
If you want to unplug the ICP sensor as Rick suggested, it's the round looking thing on the front inboard side of the drivers side head. Unplugging it will cause the PCM to assume default readings to aid in start up. It looks like this.
Now that we've gotten that out of the way, I'd like to back up a few steps. Can you give a better description of the starting and running issues? How long do you have to crank it when it's cold out before the truck starts? Is there any smoke out the tailpipe while cranking? Any smoke when it starts up for the first minute or so? If so what color smoke?
As for the running issues, it's common for a diesel to be louder when cold, and the EBPV (exhaust back pressure valve) will close to increase back pressure aiding ina quicker warm up. This will make the engine sound different, and you get that lovely jet engine sound when you hit the go pedal.
Also, just to be on the safe side, when was the last time the air filter and fuel filter were changed?
