Is it a Powerstroke or a Turbo IDI? 94 or 94.5 is the difference.
The IDI will have Ford on the engine cover.
A couple bad glow plugs may cause it to run rough at startup.
IS the check engine light on?

its a 94.5 powerstroke. and no the check engine light is not illuminated

well like Jim said could be a glow plug issue. Could also be a dirty IPR. Try starting the truck with the ICP sensor unplugged.
