A little more info on the truck would be helpfull, but off the top of my head I would guess you have one or two glow plugs not working. The rough idle could be caused by a couple of cylinders not firing until they get hot enough on their own.

Nah...pretty straightforward once you get the valvecovers off. Take a look at the "official" glow plug thread at the top of the forum page...might be some info in there you can use.
