I dont have a whole lot of experience with drive shafts but I have been told even a tiny little dent in the shaft and it should be balanced. I would call your local drive line shop and ask them what they charge to balance. I had mine done and the guy only charged me 20.00 so to me that is cheap for piece of mind.

update............

Disconnected the driveline from the tranny and brought it up to speed. No vibration. Rotated wheels and tires and reconnected driveline. Vibration is back.
I posted a question in another thread in case this gets overlooked but will ask it here too. Did these trucks come out with approximately a 2 inch block lift in the rear?????????

well, After letting another truck run for thirty minutes with the cables on my powerstroke, she finally started. I drove it to work and.......... viola, my problem is cured. so smooth it is unreal. best 50 bucks I ever spent. dude run that shaft up to 3,100 rpms after balance was complete. smoth as can be.!!!!!!!!!! NO vibration whatsoever at any speed.

Glad to hear it. Amazingly enough, there have been several trucks where the drive shaft was way out from the factory. Drive shafts are too often passed over by those with vibrations. I don't know why, but some let them go for years before they pursue them. Usually by the time they balance the shaft, there are many other worn components that are a direct result from the drive shaft. Vibrations are hard on any equipment and should never be ignored.

Just got my driveshaft back from being balanced and it runs smooth all the way to 90mph ( only did it to check for vibration ) the shop said that the u-joints that I had used ( 6-months ago ) were of poor quality and already had play in them . Napa u-joints . Anyway , problem solved for my vibration and it was money well spent !

I had this exact problem.
I replaced my tires, ujoints, had the shaft professionally balanced/straightened and had the bearings replaced in the rear end all for naught. I ended up replacing the driveshaft and it fixed it. The dealer took off the rear shaft and drove it and found no vibration. When I removed my old shaft, I noticed that if I twisted one end clockwise and the other counterclockwise, there was a very small movement in the splined portion of the driveshaft. I replaced mine with one from Driveshaft Specialist in San Antonio for one third the cost Ford wanted.

My 2001 F250 SD has what sounds like the same vibration. It feels like a U-joint when driving, but I crawled under there today and checked - they seem good and solid. I do feel a slight play in the driveshaft sliding splined joint, which I suspect could be the cause.
