i run an 1.5" shackle in addition to a 4" spring.... i didn't notice anything but a height difference either, as said above. didn't replace bushings, or hardware for that matter.
donahoe racing has theirs for $185-$199. i think its from wheelers offroad..
oh, i have bilsteins front and rear that are stock length for my excursion. i did the v code springs (2" lift) and the new 4" ford rear block (2"lift ) and the shock length is fine. i was told by bilstein that for the rear, i could do a 4" lift and still have stock shock length. the fronts, i would have to get longer shocks if i did more than 2" of lift. but even then, i would have to get a pitman arm and either a drop bracket or adjustable track bar.

I have a 2001 F250 extended cab with x-code plow springs in the front. I added a 2" Donahue spring pack to the front springs and a set of 2 1/2" add a leafs to the rear, a track bar relocating kit for the steering, and 285 tires. It gave it nice lift, and even with a full 8' load of oak firewood in the back, it doesn't squat. I think it rides great. A little stiff when empty, but not bad at all.
