make sure you have clean fuel, keep your fuel filters clean. i always warm my truck up for several minutes (about 10-15 when it's cold out. using remote start) before i drive it, and even then i drive easy for a few minutes to warm the diffs up and the tranny.
it will shift a tad rough when it's super cold, but the only time my truck would idle rough and not wanna run smoothly until it was HOT HOT HOT, was when my injectors were either bad or going bad, (i replaced all 8 with modded injectors and the problem went away completely. )
