Also depends on whether they look under it or not- if your truck's underhood emissions sticker says "Catalyst", and you don't have one, it's an automatic fail.
On the other hand, my 84 F250 sailed through CT's emissions with flying colors only because they didn't even pop the hood. It was originally a 351w electronic carb, swapped it out to a built 300-6, way overly rich Holley 2BBL on an Offenhauser intake, dual 2" exhaust, and no cats.
