A friend of mine had a similar problem as yours, it turned out to be a bad fuel pump not creating enough pressure, they also replaced his turbo.

As tight as possible... No I really do not know the torq specs but after blowing mine off a few times I have them pretty "cranked down".

Boy it sounds as if Ford really want to put the blame on his shoulders. Shame on them.

The clamps on the boots are tightened to 9 ft-lbs I believe.

Wow thats it, only 9 ft-lbs. Boy I bet I am past that. I had to replace my upper turbo hose due to a 2" crack in it. Not sure how that got there. The factory one had the clams stapled to it but the new one did not. I did noticed that each time I tightned the clamp they seemed to ride up over the "bumb" on the metal pipe. Also wish ford would of provided some sort of a bracket to secure the pipe to. It just flops around from what I noticed.

Our 04 6.0 had this exact problem where towing on flats it was perfectly fine but once you came up to any large grade it would loss power and it would only go like 30mph up the hill. I refinished the turbo and installed the new vgt actuator and it made no difference. We them thought it was the auto tranny and then decided to take it to the dealer. They had it for 2 weeks and finally were able to replicate our problem with their laptop plugged into it pulling a flatbed with a new pickup on it. After our diesel tech researched it and asked ford they came up with the replacement of the engine oil cooler and the egr cooler. The coolant temps stayed right at 180 while the engine oil temp shot up to 280-290. This caused the engine to defuel itself. Well close to $3000 later and the two coolers replaced it is pulling good. Oh the truck has 120,000 and was out of warranty. Also our tech said to stop unplugging the egr because it will affect the way the motor runs. Hope this helps
