Unhappy Fuel in cooling system

Help....I am getting fuel in my cooling system. Out of the blue, haven't changed or modified anything lately, and I removed about an inch out of the expansion chamber floating on top of the coolant. Anybody know where it could be coming from?

I'm pretty sure that fuel under pressure is getting past an O-ring on one of the injectors and winding up in the cooling system.

Is this a 6.0 or 7.3? I'll move this to the right forum for you.
