I have never mounted the spacesaver on any of my last 3 cars. I've had a couple slow leaks due to punctures (nails, screws) but they never went totally flat before I noticed them. I do have a "gooey string" puncture repair kit and a 12V air pump with me when I travel long distances. Total blowouts are very rare today. If you use tubes and/or bias ply tires tho, good luck to you...
B/B Ford, the old Cragar ss and Keytone "tri fit" wheels ('cause if this doesn't fit, we'll tri another one) had 5/4.5, 4.75, and 5/5. You had to use a long shank lug nut with an oval washer. They were a pita, but would solve your problem. You see old used ones at used tire stores sometimes.
Just another thought for those of you carrying a spare...
...make sure you have the right lug nuts for the spare. It would be a drag to get the danged thing jacked up and fit the spare on only to realize that the mag wheel square shoulder lug nuts wont work with the conical lug nut style steel wheel of the spare. Migth be a good idea to toss a handful of regular lug nuts in the travel box.
