what are you guys running for cat-back pipes? I do not want to remove spare for pipes, also just want some sound but not too much as it is the wife's truck. 04 SC 5.4
I have seen them 2 ways. One way is side by side out the same stock location behind the passenger side rear wheel. I have also seen them straight out the back under the bumper or out the rear corners. If you got a good exhaust shop that custom bends pipes it shouldn't be any problem with the spare. I had a custom setup on my Expedition with coil suspension and side by side duals. The shop did a really nice job!
I had a shop cut my original exhaust of at the cat and run stainless pipe back to a stainless magnaflow SIDO muffler, then mandrel bend stainless pipe over the rear axle so the duals came out on each side behind the rear wheels. Like the stock but dual left and right sides with 3.5 inch slash cut poliished stainless tips. Sounds nice looks nice and no trouble with the spare or exhaust blowing back to your trailer if you tow. Cost $525 this is on a 2005 screw 4wd
I opted for a single in dual out rear exit cat-back kit from Borla (#140087). It's bent specifically for our trucks, has a nice but not too loud sound, is 100% stainless steel, and not cheap. Pics and a soundbite are in my gallery.

IF the exhaust system is designed correctly you can leave the spare in place. What good is a vehicle with no spare. I wouldn't install any mod where I had to lose any function on the vehicle. The spare is definately a needed function!
