The bolt patterns are not the same, and there may be clutch issues as well, but like Dave said...there might be adapters available. Is using a Ford five speed not an option?

It may be, but this is the tech forum for the 6.9 and 7.3 IDI. The only places the merits of the 5.9 should be discussed are the general diesel and Ford vs. the competition forums.
im not up pn the interchangability of these engines with the trucks oem tranny/clutch/transfer case, but i do know how a pain some of this stuff can. I think that you would be better off using the whole ford setup (engine, tranny and tcase) . Im not familiar with the late 80's chevys, but if there is room for the whole ford setup, a couple driveshaft changes might be easier then mixing drivline compnets from diffrent oems.
I saw a chevy with a 6.9 in it on ebay just a week or so ago, but they didn't show much on the powertrain, so I don't know just what all it had in it. The bell housing is going to be the biggest problem, as the diesel is all by itself on interchange. I doubt there are any adapters readily available for that reason, and lack of real demand to do it. I think you would be best served to use the Ford trans, and then get an adapter for the t-case instead. Since the t-case has the driveshaft on the same side, you could use the Ford t-case as well, and then just make up driveshafts for it.
I think I would sell the Chevy and just buy a ford . If it was a classic truck , that would be a different story . I have seen an older Chevy crew cab 4x4 with a 6.9 .Not sure what tranny and transfer case he used . He may have changed the front axle .
Technically, they are IH, but many people don't differentiate that. Ford hasn't built their own diesel for since the 70's, except the tractors, and even those I'm not certain about.
