I have a 5.4L. I started my truck up yesterday morning after sitting over 24 hours and it started fine but when I took off it just sputtered and would hardly go for at about 20 seconds then ran fine. It did it again this morning. So I lifted the hood and found nothing wrong. I ended up taking the airfilter out. I just happen to notice there was a huge hole in it. The air filter is shaped like a cone with a cap on one end. The cap is glued to the paper filter material. Well about a quater of the way around that cap the glue came undone allowing unfiltered air in. This filter has been in there for only 6000 miles. I acutally put a Fram in there replacing a K&N thinking I would reduce the dirt coming in the engine. So after throwing a minor fit I went and got a Napa filter. Anyway the truck runs OK accept for 2 things. The startup issue and it also has a hanging idea. When I shift gears the idle goes up and it takes for ever to settle down to the normal idle point when the truck is stopped. I don't know if these problems are related but I won't be buying any Fram airfilters agian.
Fram = "Orange Aisle of Death" at your favorite store. Bad filters of all kinds, built to a price point. Which is why ChinaMart (WalMart) carries them in addition to our local chain, Meijers. They may have been decent years ago but now they are the "bargain brand" you put in vehicles you lease.
I think your right. The truck did hesitate once agian when it was cold this morning but for on about 5 seconds and the high idle is gone. I looked over all the vacuum lines last night and saw nothing wrong. I guess i'll have to do a more in depth inspection this weekend.

Like I said, with unfiltered air entering the intake, check the MAF for dirt.
My truck has had the same issue with sputtering at times. Usually happens when it sits for a few days in cold weather. I tend to think fuel is more the problem than a air filter.
Check the IAC (Idle Air Control Valve) on top of the engine. Clean or replace the valve and that should take care of the idle/stalling problems when cold.
To you guys that think Fram is so bad, next time you are in Wal-mart, open the box on a Fram oil filter for a PSD. Open the box on the Motorcraft oil filter for a PSD. The Filters are just alike. Same patent number and all. Both made in Germany.

Why just a PSD and not gas motors? So you are saying the germans are bad?...lol
