How long has it been since your injection pump was rebuilt or replaced? Do you change your fuel filter often? Is there an air leak around the fuel filter? If your fuel filter is seeping air it will cause terrible hesitation and rough idling no to mention a gob of white smoke out the tailpipe and will also diminish your fuel mileage. Had this happen on a 1993 F-250 once when my not so bright boss put both rubber gasket rings on the top of the filter instead of putting one on the bottom for the water separator. Didn't take me long to figure out the problem but the truck barely ran and was billowing white smoke from the tailpipe. Check the fuel filter and if it has been more than 80,000-90,000 miles you may want to have your injector pump rebuilt again. Often times they wear quickly and need rebuilt. Also check for a cracked injector line or a bad injector. Hope some of this helps!

The truck will not idle, will start ok, but won't idle, also mileage has gone out the window

On your injection pump linkage there is a screw with a spring around it,(basiclly its just a stop for the linkage)that screw adjusts the idle.Check it to see if its backed off,or just turn it a couple of turns and see if it will then idle,if it does wait for the engine to reach operating temps and set your idle with that screw to 650-700rpm's.How much of a drop did your fuel milage take?You can expect it to drop a couple of mpg's when the stations start selling their winter fuel.
No haven't used any additives in this one, have run some sea foam through a couple times, mpg has dropped to about 8, I am leaning towards the fuel pump needing rebuilt.
