my 99 v10, with a 5spd and 15000 miles keeps slipping out of 4th gear. Once in a while, it grinds downshifting into 2nd gear. Won't go into reverse rarely, without going into first gear, first. Basically, it feels like its already got 100000 miles on it already. Any one else had this trouble. On a side note, I shifted periodically without the clutch, with good success.

That sounds like a bent shift fork type of thing.
Maybe its not getting completely into 4th.
As to difficulties shifting into reverse, (having to shift into 1st first.) I've heard of others having this problem as well.. Sometimes I have to do this too.
Grinding into 2nd:
Remember 1st and 2nd aren't syncronized, so you basically have to be almost stopped to shift from 3rd to 2nd..
As far as shifting without using the clutch, thats your choice. It takes a real expert (ie someone who drives large trucks for a living etc), to do this consistantly and not overwork the syncronizers etc.. I'm not really sure this tranny was really designed with that in mind.
I got 190,000 miles on my 89 ranger 5sp, and never had any clutch or tranny problems.. In fact the tranny shifts smoother the older it gets it seems.
I believe my SuperDuty clutch/tranny will hold up as well. I use the clutch when shifting.
I have had two sets of syncro's replaced. Ford dealer did them. I have been driving sticks since my first f-100 with a three on the tree. I have no idea what's up with this. Fluid is ford spec and to the required level. It is most embarassing to hear the gears grind when the clutch is on the floor. Not to mention expensive. BTW this was in the span of two years.
Welcome, but you didnt say much about what kinda truck you have, miles, motor, etc. I do know there have been problems with synchros with some of these units. Sorry to hear. My F150 5 speed is trouble free now with 150k on it, changed the fluid once. I hope my 6 speed is as reliable and for longer...
My 2001 F250 SD extra cab 8 foot bed with 4x4 had 50,000 miles when I bought it. About 62,000 the first time they replaced them and 13 months later they started going again. I haul a 12 ft. dump trailer, 20 ft flatbed trailer and use it for a hvac/r service truck plus my personal trans. The fluids are checked atleast once a month and havent dropped since the original repair job.
