Been chasing a fuel injector problem for a while now due to fuel knock and excessive hazing at idle.
Ended up changing all 8 injectors at the end of last year, seemed to fix the problem. IQAs were programmed, injector values were reset, and crank adaptations were reset. Injectors were changed out with Ford / Bosch remans. Truck ran great for about 2500 miles.
Truck is back to throwing minimum offset learning codes (injectors 1 and 6) and an injector with a -4 or more balance rate (injector 6). After resetting fuel adaptations again, it only takes 3-4 start ups for the truck to start fuel knocking and hazing at idle again, When resetting adaptations, injector 6 "starts out" with ~ -2.5 balance rate.
Could this just be the case of a bad injector again, or am I chasing something bigger?
How many miles and is truck stock or deleted? When you mention hazing I’m thinking deleted.
Be curious also about the difference between commmanded vs actual fuel rail pressure.
That would be an indicator of cp4 health.

Agree, I'd pull both the PCV and VCV and check for contamination. If you don't find contamination, Check the values KOEO, at idle and under hard acceleration. If they look wonky, you may need a new driver side rail or HPFP. Go with the DCR if you need a pump.
Truck is deleted with a 75hp AMDP tune on it. No fuel system contamination, checked both the metering valve and drained/ changed my DPK filter. Commanded vs Actual values out of the CP4 are practically identical.

Have them write you a 0 HP tune and send it to you. I've seen a bad file have folks chasing their tail.
Something else that occurred to me is that you may have an electrical issue occurring. Check the pigtail for the injector and see if wires are chaffed, connectors are damaged or corroded. The connector may have been replaced and someone may have repaired it incorrectly.
Something else to check is that there are NO AFTERMARKET SENSORS on your truck. People replace the sensors on the driver side rail with aftermarket parts because the Ford ones are expensive. The same goes for the rail pressure sensor and MAP sensor.
It may also be worth swapping the injector again, I've seen where they go bad. MAKE SURE the copper washer came out with the injector.
