Overflow tank cap blew off

Overflow tank cap blew off

Hi all. I hope everyone is having a good start to summer.
This one is new to me, so hopefully someone has an answer.
2013 F450 platinum dually 6.7
Went to pick up our camper the other day. Prob 6000 lbs
There is a long hill that is about 10 miles and approx 2000 feet of climb.
I was just cruising and all of a sudden about 9 miles up, I saw smoke out the back. It was actually coolant.
Found a pull out location. The truck never fully overheated. The gauge did go up, but not all the way.
When I lifted the hood, the overflow cap was off and just sitting there. It had blown off.
Of course I had approx 1/3 of a gallon of coolant to replace and the rest water.(this is all in about a 2 hr period as we had to get our son to come save our butt.
Refilled everything, he towed the trailer home and I made it home with absolutely no issues.
What the heck could have happened?
Ive got new Ford coolant ordered as I have 80% water in there now.
Ive never really had any probs with this truck at all.
There is a check engine light on for the DEF heater BS that these trucks all seem to have. Truck drives great, but I do know that I need to fix that.

Sorry for the long post. Thought the more info the better.

The primary system cap is threaded on, so...... wasn't tight??????

Worst case is over pressurizing of a cooling system can be from a bad head gasket. This can pressurize the cooling system and blow hoses or?? Bad rad/tank cap, not tight as mentioned and vibrated off maybe.

Sounds like human error left the cap loose and it vibrated off. To blow off a threaded on cap something else would have let go first if it was tight.

It has been 3 years and lots of towing since I have topped off coolant, so I am leaning against human error, but thanks, I know how to screw on a cap.

The fact that I had to top off 3 years ago makes me wonder why I had to top off.
Ive always thought the smell of my exhaust was a sweet smell. Almost like there is coolant dripping in.
No smoke fro exhaust.
My oil is not milky.
The truck runs great
I sincerely hope it is not head gasket.

Keep them fingers crossed its a fluke. Might try a cooling system pressure test/leak down.
