If you fix it, you'll get a few more rainy seasons out of it without water spraying into the cab when you hit a puddle, or water leaking into the bed or cargo leaking out.
It does not appear that your frame, suspension mounts, or the general integrity of the body/bed are compromised. Thus, not quite ready for the scrappy yet, unless you have a fubar drivetrain that's beyond repair--which would be unusual.
If you don't fix it, well, you'll slide into that zone where you sing Adam Sandler's "POS car" song to yourself as you drive.....
That doesn’t look too bad. You could cut that out and repair it. The rest looks good to me. Was there something wet sitting in there, or a water leak and holding moisture? Looks pretty localized.
If you’re not up for welding under there, you could probably get away with cutting out the rust, fabricating a small patch, and panel bonding it place. Use a few sheet metal screws to hold it in place while it cures. Either cut off the ends that protrude into the cab floor, or take them back out and fill the small holes with a little more panel bond. Brush some primer and paint on, and carry on.
Oil / Woolwax spraying, or painting under there will help. Oil spray does make a mess to work with in the future, but it stops/slows the rust.
That would be considered a clean rust free truck in the salt / rust belt…

This is the only rust on the entire vehicle. That's why it shocked me to discover.
The cavity was full of wet dirt. How can dirt get into the rusty area?
that is patina. you want to see rust, look at a truck that spent its life in new york state or new jersey where they believe the more salt on the road in winter the better the roads will be.
