Over boost then reduced power

Over boost then reduced power
Not mine
Nephew has a 21 F-250 with 6.7
On his way back to ND pulling 36’ enclosed trailer, says at random going up a slight grade with on extra fuel increase boost will start creeping up from 15 to past 40 psi, then it will trip a code and defuel
If he stops and shut the truck off then restarts everything is ok and he can proceed till it does it again, could be 100 miles or more before it happens again, if he sees boost creeping up and let’s up on the pedal for a moment he can get back into it and boost will return to normal
Is this possibly a sensor malfunction or vanes in the turbo sticking or another issue
PS: He said before the boost starts creeping up the exhaust brake light turns off
