I was replacing a bunch of stuff last week chasing a drop in MPGs and noticed this open nipple on the DS valve cover. Does anyone have any idea what's supposed to be connected here and where it's supposed to go to? And any idea if this could this be related to my drop in fuel efficiency? It doesn't look like there's been a hose on there for awhile, and I definitely didn't take it off. (It's a 2001 E350, btw. I've owned it for about 2 years and 14K miles.)
Looks more like a wire loom grip post to me, not a hose nipple. Example is right below it in the photo.
Most likely the case, with engine running put a hose or low pressure gauge over the end to see if vacuum or pressure is present.
When were spark plugs and boots last changed or at least inspected? What's your fuel pressure at idle and at freeway speeds? Have you done a scan checking for both short term and long term fuel trims? What was MAS cleaned? When was air filter last changed?

All 10 plugs and coil boots were replaced about 12k ago, along with fuel and air filter. PCV valve and hose, IAT and TPS were replaced about 5K ago. Both o2 sensors were replaced about 2500 miles ago. MAF sensor was cleaned about 10K ago and then replaced about 1500 miles ago. All 10 coil packs and air filter were replaced about 500 miles ago (all Motorcraft parts), and the throttle body was removed and cleaned thoroughly. I only use top tier gas (usually Shell), and have regular oil change intervals of 7500 or less, using Mobil1 and Motorcraft filters. Last summer on road trips, we were getting 13–15mpg, then it dropped drastically to <8mpg. Replacing the coils and air filter recently got me up to 11mpg on a similar road trip, but I know she can still do a lot better…cause she has. I do NOT drive this thing with a heavy foot at all—I have turbo-charged, manual transmission cars that scratch that itch. This is our family adventure wagon on school breaks, and neighborhood carpool party bus during school. We've got a 3500 mile road trip coming up, and with gas prices where they are, I'd LOOOOVE to squeeze back into a better mpg, if I can. I've read through SO MANY threads on V10s and low fuel efficiency. I know some of that is just par for the course, but like I said, I know I can do better than I am/have been.
I had an intermittent misfire before replacing the coils, but outside of the exhaust smelling like it's running rich, it idles and runs great. I'll try to check fuel pressure at idle and freeway speeds, and check short term and long term fuel trims. I'm not sure what I'm looking for, but I'm 95% sure my scan tool can collect that data.
that looks like a valve cover bolt, not a hose connection I just check on my motor, and that's what that is a valve cover bolt that nipple is in case you need to put a wire loom on it and keep it out of the way
