I think your question is right on target, though I don't have an answer. I'd guess heat, but that could be either coolant temp or, I suppose, cylinder temps, but I don't know that either are involved.

Or air in the fuel, making the equivalent of a "water hammer" effect like when you turn the water off suddenly in an older house?

With the right tools it's not a bad job. Honest the first thing I ever did on a 7.3, mine, was injector cups and with the Riffraff tool it went smoothly. I went for the stainless cups, gave me the warm fuzzy that they won't crack but that's one of those things that's only true until it isn't.

Pull the heads is my recommendation. At 240k your valves are most likely crap anyways. YOU CANNOT go by looks unless you've had it since new. I did mine and the left bank was like new, so I knew it had been off at one time. 3 hr drive each way fro me to find a machine shop that would do the valve job as special tooling is needed.
I would never do another high mileage injector swap again. If I get to 100k on this rebuild, then cups will be done. 250K+..never again
