I have a 2002 Mazda B3000 V6 Auto with 110,000 miles. After driving only a few miles, the O/D Off light will start flashing, the truck will not shift into OD, and the shift into 2nd gear is a little rough. When I stop and turn off the ignition, then restart, the light will be off, shift normally, but start flashing again just a few miles later. I read that the speed sensor on top of the transmission is often the problem and changed it. The OD light was off, everything fine on a 10 minute test, then started flashing the next time I drove it.
Or...you could read the codes that are set when the OD OFF light flashes. This will tell us exactly where to look.
The places that read codes free cannot read these codes. I recommend Forscan, available free at www.forscan.org. Their website tells how to select an OBDII adapter. I bought mine from Amazon for less than $30. Or you could take it to a good independent shop or a Ford dealer to read the codes.
Where is your OD on off switch?
If it is on the column like the ford trucks and vans
The problem may most likely be broken wires at the base of the shift lever
Pull the column shrouds and the boot surrounding the shift lever
Inspect the wiring close
I have replaced so many I have 1 in my toolbox
