Mine are factory installed so if nobody has something better I can measure them for you. you'd think there would be a factory diagram or something though that might be easier to follow.

Never seen OEM "in bed" tie down rings in a dent.
I just first put mine on top of the bed at at standard holes after removing the OEM rails in the '80s, then I put a shell on it and put black rubber ribbed pieces on top, and when the shell came off, I simply drilled 6 holes in top of bed sides and mounted my chrome tie downs using loctited studs in bases and a nyloc nut and washer underneath, they can be rotated.
I installed two sets of d-ring tie downs in the “post” at the tailgate end of the box. One set is as far up as I could get with a heavy fender washer inside the post, the other set is installed with the d-ring an inch or so up from the bed.
It was fiddly getting the washers and nuts inside the post, I don’t remember exactly how I did it but it may have involved removing the tail lights.
You can’t see them in the picture but you can see the angle of the strap and why I chose to mount a set low down.
I’ll try to get a pic with measurements tonight. I find them very handy. Since I don’t often carry tall things and don’t like removing the canopy, tie downs along the bed rail didn’t seem as useful.

I haven't yet but probably will. There isn't a boxed section at the front but there is enough space between the inner and outer walls of the box to get a hand in to hold the nut while starting the bolt. It will be a two person job. Might even use serrated lock nuts since you don't really need a wrench to get them tight once they are lightly snugged. That way, I can tape a nut to a stick or stiff wire to get it up fairly high for the upper tie down.
