I expanded the picture and it looks like it's rusted through the roof panel so it would need a new roof skin and if it's down in there to far the roof under structure as well plus a new windshield. Depends on what the truck is worth to you how far you want to go.

Truck has lived a very hard life, had it gifted to me. 99 F350 7.3 4x4 dually. I just paid DMV fees, registration, and miscellaneous maintenance/parts. My plan was to use it as a work truck. Get it mechanically solid, fix the windshield rust, and have Maaco shoot some cheap red paint. However, I'm not sure how much it'll cost to repair the rust properly. I was thinking I'd just grind it off, but your thoughts sound much more expensive lol
I agree with RVPULLER... its a 20 year old work truck..... I would get an electric wire brush and a good die grinder.. CLean out all the rust and crud and grind the paint back 1/2 inch onto the roof to good material... I think a good red ozide primer over the are would be the next step as a "base"......... your going to have a few holes... I would get some Silicon caulk ( can be black) and put one layer around the window and down in the holes to seal it up.. Let it set overnight and put another layer over the top .. spread it smooth on the top with a putty knife... It can be an inch wide, you dont care. .. get it sealed to the window and the roof panel.. everything in between gets filled....You can probably do that for $20.
I’m finding much cheaper prices than US body shops for bodywork and paint using equal or better materials. Got one quote around $2500 for a full truck repaint using PPG paint, including five hours of rust repair. Every shop around me wants to do the project using insurance, not cash. Getting quotes closer to $7000 for similar work. If I don’t end up doing the repair on my own, it makes financial sense to take a field trip over the border. But we will see.
I'd re-skin and do the pillars -- parts are readily available, you can do this yourself, and the aftermarket sheet metal should run around $300 or thereabouts.... better stuff (and cheaper) can be cut from vehicles in the junk yard............pull out the Blue Box (It's Milla Time) and watch the heat.............
