would like to modify the sliding door on a 1989 e 350 to a single door that swings open 180 degrees.
Has this been done with the existing door?

What benefit would this provide? You couldn't open it in a standard parking lot with another vehicle in the next spot, and opening it on a windy day could get interesting if the wind catches it. I'm not a fan of these sliding doors but they are definitely the most stealthy option for use in tight spaces.

Instead of moding the slider we would like a door that opens 180 degrees and then we could add cupboards ect to inside of door that would open with door,
this would increase overall storage capacity and you could access from outside when doors are open without having to enter each time .

Damn.. never thought of that one, yet another reason to dislike these doors. That is going to be a heavy door which should probably have a gas cylinder to dampen and limit movement, but if you're adding a cylinder why not make it powered.
Slider stickout extension: I tried to find one, they exist or could be made. You'll be on the road in no time.

Single door? You will have to reinforce and beef up the hinge side, as well as the door. And you will have to engineer or cut and weld a door latch system.

Plenty of custom vans with the door that opens up. You couldn't do the shelves in the door but it would be super heavy for the hinge side anyway. I don't see the metal being strong enough for it to last too many years. Those that open up may be fiberglass. If you could use one of those it would be much lighter and easier on hinges.
