Starter went out and replaced with new Denso model from O'Reillys with lifetime warranty. Seems to start OK 90% of the time, but then it'll get on a kick where it just grinds against the flywheel (spins and makes awful noise, but won't engage). This is my second Denso because I thought it was a bad starter, but turns out it's something else. Any ideas on if I should shim it or something? Right now it's stranded in a parking lot giving me the sad face.

No shimmering should be required.
Is the starter a true Denso or a knock off like DB Electric? Just like with Motorcraft sensors, sometimes there are big benefits in paying for the genuine part.
Based on what you have said, everything else in the starting process appears to be functioning properly. The starter is probably bad, which is why the parts stores often offer lifetime replacement. I have replaced my AutoZoo steering gear twice under warranty because the OEM gear failed when we were on a camping trip and I could not wait for a WTO or Blue Top gear.

Last I was in there looking for a starter they don't carry any Denso starters for these engines. They do carry generic Denso starters and there is a big difference. Make sure the part number you got was DENS-TG228000-8420
The Bendix on the starter is not being thrown hard and fast enough into the flywheel for some reason. Take the starter off and clean all cables for corrosion and clean where the starter mounts. Take the starter to Oreallies to check on how it spins and throws the gear out and is not hanging up or hesitating. (batteries fully charged and good)

I bought my Denso from Corey at CNC Fabrication. Great guy to deal with who does what he says and says what he will do.
Thanks guys. Good to know I don't need shims, bad to think I might need a flywheel. Bolts are tight with blue locktite. I'm going to pull it again tomorrow and clean off the mating surface real well and see if that makes a difference. Once I get her home (tomorrow now, ran out of time today) I'll get some pics and see what's what. The old starter was fantastic until the solenoid went out. I should have kept that sucker, but I had a dumb moment. It happens more often as I get older.

Using a 10 gauge wire for the trigger wire will help. You can also adjust the turn on distance by removing the rear gasket on the solenoid and making a thiner one. I have added an ajustment screw to the back of the solenid before to do the same thing. It is likely spinning to early.
Had an issue a few weeks ago, truck wouldn't spin very fast.... no start. Pulled batteries and they tested Good. Over the weeks, several times truck just didn't sound right starting. Looked up my starter in my records, and it was over 5 years old. So I go crawl under and as I pull on the cables, etc.... the solenoid almost fell off in my hand. Glad I wasn't out in the national forest somewhere...
Advance Auto.... Lifetime warranty... so the new one was a freebee.
I had issues with knockoff Denso starters that had me pulling my hair out, finally went back to OEM and no problems since. There are a couple threads around the net on Chinese Denso starters, I wasted a good part of my life on it before I found out the starters were junk.
The ‘denso’ from autoparts is NOT the same as the one from CNC Fab.
Get the real denso starter and never buy another starter. Rebuild it yourself forever. Mine needed new contacts ($30 and 10 minutes) after 180k miles and is over 280k Miles now. Eventually it will need bearings and someday the motor may need replaced. All easy to DIY.
Meanwhile, it spins the engine faster and draws less amps than the other options for our trucks.
Ive put lots of these in and mine is the only one that needed service (10yrs later).
A drop of blue Loctite on the starter bolts is a good idea.
