Have you checked for wire chaffing near the 42 pin connector? Where is crosses the valve cover?
I have checked that and ohmed everything out from the IDM connector, but because that seems like a very valid possibility I am going to check it again to make sure.
After 5 months of chasing this issue down it turn out the catalytic converter was clogged up enough that it was preventing the engine from burning all the fuel due to the back pressure it was creating. I disconnected the exhaust and all the smoke went away. Replace the catalytic converter and the truck is running nicely now. Thanks for all the suggestion and help I received from this forum. It is great to have such good help when you need it.

You can always "unclog" them using a pressure washer back flushing them. I did this to a 6.doh kitty thats in my OBS as we speak. That sucker was plugged solid.
