Have you been able to get the codes? Did anyone take the HPFP apart and see if it failed? I'm no diesel mechanic have been doing a bunch of research on the HPFP, I haven't read anything around a failure resulting in the symptoms you're describing but it could be metal shavings in the injectors if the HPFP failed or was on the brink of failing. It sounds like the only part of the fuel system that you haven't changed are the injectors. Any idea where the blue plastic ribbon came from? Was it in the filter or in the line? I would agree with KPER, I think at this point you need to get the fuel pressure. Also as Dave mentioned, we've seen more than a few occasions where the little cardboard piece inside fuel additive caps have fallen into the tank and caused problems like you're describing.
