13 is the idle speed control device did not respond correctly.
That sounds about right.
It idles pretty good, loads up a bit, then goes back to idling.
But that surely isn't the problem.
It still runs a bit rough, and a fair amount of black smoke/missing.
I thought the three digit codes didn't look right, didn't trust my memory and should have. Internet failed me. Looking in book now at two digit codes. Shows 13 to be rpm not within self test lower limit. It's a koer code. Shows 11 to be pass(separator code) and 31 is koeo, koer, and continuous memory for the EGR valve position(EVP) sensor out of range (low)
Well, I did break that little green vacuum line on the egr, but I still think my problems run deeper than that.
I'll have to look around for some vacuum hose to replace it with, but then I'll probably snap a few more in the process.
Ok. I'll see if it spits codes.
I'll probably get writers cramp by the time it's done.
One of the mechanics left two boxes of plugs in the truck.
As I prepared to pull and replace them, it appears that has been done already.
Who puts the old plugs back in their boxes????
I do also but because it is easier to carry them in 1 big box than 6 or 8 little boxes and all the tools.

Clear all the memory codes , disconnect the battery pos and turn head lights for 30 seconds then shut off. Reconnect the battery and start the truck and let it run for a few mins rev it up a couple times turn the heads lights on an off once or twice. Shut it if off and pull the KOEO (Key On Engine Off) codes once again.
Let us know what you get then...

You'll have to excuse my lack of...something.
What does turning the head lights on do when there's no battery connected?
And again, on and off while it's running?

Head lights on with the battery disconnected will drain any residual voltage stored in capacitors that may prevent the ECU from resetting. It is unlikely but still a good practice when using a battery disconnect to reset the ECU.
Headlights on after a start will load up the alt causing the engine rpm to drop forcing the IAC to adjust the idle RPM the same thing can be done more effectively by turning on the AC but was unsure if your truck had A/C.

Correct me if I'm wrong but the easier methed is to unhook the battery for thirty minutes or leave it unhooked over night, say right before bed so no down time.

Not sure how letting it sit over night or 30 mins is easier than disconnecting the battery pulling on the head lights for half a min or so then turning them off and hooking the battery back up. It literally takes 5 mins to accomplish this.

Because you can do it in your sleep? I had actually never heard about turning the headlights on, going to put that in the box.
