Only two: transfer case and fuel tank. The transfer case skid plate is about $30 online (before shipping) and I don't believe it comes with bolts. The fuel tank skid plate is quite expensive and runs $130 - $200 online (before shipping and installation bolts).
Also, I think a small child could easily install the transfer case plate. The fuel tank plate, however, isn't easy to install. Above, people mentioned that you don't have to drop the fuel tank to install it. I read somewhere else that the mounting locations are so tight that it's nearly impossible to fasten the hardware without dropping the tank at least a few inches. I don't have any personal experience, so I don't know what the real story is ... other than, no matter how you slice it, installing the fuel tank skid plate sounds like a real knuckle buster.
If you value those two skid plates, the $90-ish for the two are maybe the best deal/value on the entire factory options list.
I was driving across a pretty snow covered abandoned parking lot once and a huge piece of rebar was sticking up. didn't see it but my skid plate bent it over vs hitting my aluminum oil pan on my 2500HD 6.0 Gasser. 100 bucks was well worth it for me.
Has anyone ever actually had a skid plate hit? I've got a soon to be 19 year old SD and have driven in everything but rock crawling, never needed a skid plate.
I've had skids plates on multiple vehicles and never had to use them (as far as I know). I'm more concerned about kicking up a chunk of metal or whatever on the highway and damaging the tank or transfer case. For me, I look at it more as insurance than something I'll use every day. A punctured fuel tank while on vacation would certainly be a pain in the ars.
