I've got a 95 F-150 4x4 5ltr auto. It developed a noisy T case after installing 2 larger tires on the rear than the front, causing the 4 wheel drive to not disengage. There was a lot of metal in the T case oil and the noise.
I replaced the T case with a used one that visually checked out, but still have the driveline noise, better than it was. I pulled the rear diff cover everything looks good and tight.
It is noisier when letting off the gas, like it would with a bad diff.
I'm going to run the new case for a month and drop the oil to look for indication of another bad T case.
Any other suggestions would be appreciated.

Fixed the tire problem but not quick enough. I didn't think there would be enough difference between an Lt and a P rated tire in the same size.
Noise is the same in 2 or 4 wd. I pulled the original case apart, the internal magnet was a ball of filings, but I couldn't see any real damage.
I've run a P rated spare tire with three other LT tires for a few thousand miles, all the same size. Never caused the transfer case any issue. I'd be leary of trusting that to be the cause of your grief.
In addition to the advice above, a couple things come to mind; I'm just thinking out loud here.. Have you inspected the rear diff (edit, I see that you have. Pinion tight?)? Do your front hubs disengage when unlocked? Could the shift linkage for the case not be completely disengaged when moved to 2x4? Do you drive in 4x4 on pavement?

Check this. I thought I had a problem with a whining trans, turned out I hadn't pushed the lever far enough forwards. I happened to catch my big dumb foot on it one day and clunk I went forwards and no more noise.
I just replaced the transfer case, so not the answer.
I've owned a bunch of these trucks but not many automatics. Could it be the transmission?
The noise is getting worse, I just wish something would blow up then I'd know!
I found out the transfer case does not disengage when pushed out of 4Hi. With the hubs locked it stays in 4 wheel drive, and doesn't disengage when the hubs are unlocked.
Does anybody know if this is a linkage issue or something inside the transfer case.
I'd say linkage. Start by disconnecting the linkage from the transfer-case, then try to get it to 2wd by hand. See if it disengages, if so you know it's in the linkage.
