It should just be a phillips screw in the middle of the crank mount. It should be readily exposed. You just undo the screw and pull the handle straight off from where the screw was. Sometimes a slight wiggle is required to release it from the splines.
You don't specify a year, but some of the newer trucks that still had cranks utilized a c-clip behind the handle. Best way to get those off was just a small towel or cloth behind the crank mount would usually pull it off.

There might be a burr on the end of the shaft. Also, if you don't have one already, panel clip pliers are a good tool to have to remove the panel.

I had one some NIMROD epoxied on because the splines were stripped in the potmetal crank base - I ended up using a longer screw for a center for the puller screw to push against, then a puller with a bearing-splitter base (trying to not damage the door panel), but you might be able to push the door panel in enough to use a jaw-type, or even a battery post puller might work.........
