I've never seen them from anywhere and if you have one damaged it's probably wise to replace the whole injector in case of internal damage that might have occurred. That's not something you want to chance IMHO.
If you not the one that put them in could of been the person that did.
When I under torqued one of my injectors and it had O-ring for lunch
I had to carefully brush and apply carb cleaner to flush out the particles
from the screens. I was worried about damage so I was really light on the
little brush. Took quite about to get it cleaned up.
Now on to how yours could of been damaged. If somehow you had some
debris in the fuel system that got past the filters and into the head. I do
think 60PSI could push a screen in it the others are also plugged up.
In all cases where a screen has failed I would just replace the injector to
stay on the safe side. You never know what any fragments will do.
