I've heard too that Ford didn't make a 4WD crew cab dually until the Super Duty in '99.
As far as seeing one that doesn't look like a conversion, it's pretty easy to convert a 4WD crew cab SRW to a 4WD crew cab dually. Rear axle and bed from a dually truck (or jest get the fenders and add then to the original bed), and then a little work / parts swapping on the front spindles / hubs (not sure of the details) to get them to work correctly with dually wheels.

No, they never did make a dually PICKUP 4x4 until the super duty in 99. The axles you are seeing swapped in are from CAB and CHASSIS dually trucks which they did make in 4x4. Cab and chassis trucks are different animals from the cab back, but their dually type dana 60 axles will fit under pickup trucks.
So a crew cab f350 single rear wheel 4x4 will have a dually PICKUP axle swapped in the rear, and a CAB and CHASSIS dually front axle swapped in the front, or just the outer dually wheel parts will swap onto a single wheel dana 60 axle. You can also swap the dually dana 60 frontend pieces onto a dana 50 ttb axle.
A crewcab dually 2wd will have the modifications necessary to put the cab and chassis front dually axle under it, it's not a complete bolt in, but can be done.
