It is not recommended to start the engine in high idle, especially if it is cold. You want to get oil flowing a bit before bumping the RPM's up.
I am not sure the extent of damage you could cause from starting the truck in high idle, as I haven' experienced it. I would assume it could cut down on the life of the engine if you did it continuously. If you accidentally do it a time or two, you will likely cause no issues.
I never start in high idle. I have the ACPM and when I start up I let the engine warm a little bit and make sure all of the fluids are moving and circulating and doing what they should be doing, then I will push the button to ramp up the RPMs.
