Yes, it can. If it's on the passenger side, it's particularly difficult. My very first job with my Timesert kit was a #4 on a 99 5.4L. Took me the better part of a day. My alibi is that I was still recovering from surgery (BIL TKR), but that rascal is TOUGH to get to back there.

As a frame of reference, it is easier to do a Time Sert repair on a back plug in the truck than it is to pull the head off.
Even if you remove the fuel rail and heater core hoses, it is still easier than pulling the head off.

Yes, indeed! On that particular one, there was a hard heater line that came out of the the top of the engine that, with its 90° angle, that interfered with pretty much everything. I had to remove heater hoses, the electrical block with several connectors, the fuel rail and probably more.
