I just had the same problem, tore the boot trying to pull it off but didn't notice the end was missing when I threw it away.
It took hours and hours of picking at the rubber to get it all out, and I ended up using a USB camera hooked to my laptop and a couple crude tools I made.
They look like hell, but eventually I got the job done.

The top of the tube is crushed so I could drive it with a 3/8 ratchet... the first one I made out of a 17mm deep socket only to realize it wouldn't fit in the hole.
The hard part was getting the piece of the rubber boot back up over the hex on the spark plug; after that I just picked the pieces out with a medical forceps thingy.
