The round part shown below is a threaded plug that fits into the bottom roll of the tailgate. It can be welded in place, or a small locating hole can be drilled in the plug and bottom of the roll, so you can insert a punch or awl to hold it in place while installing the bolt in the end.
Yes, they could easily be fabbed up by someone with basic fab skills.
I use them all the time but I don't install them as per included instructions. I weld mine on the INSIDE of the tailgate flange and trim the tailgate as needed to fit them. The end result is that the weld-on tab is much stronger and the whole assembly is more "hidden". Here is a pic of them installed on my 56 bed before paint...

I bought a set of hinges from CJpony for my tailgate and the diameter of the outside of the hinge is is the same or smaller than the rod that the tailgate swings on. The bar is way too big for the hinge. Perhaps the bar was put in by someone else.. any suggestions?
I know what he means about a round bar. My tailgate had a bar in the tailgate when I bought it from Grandpa in 1977. Whether the tailgate was fixed by him as a farmer or if he took it somewhere. There was a homemade "hinge" below the tailgate that the rod went though. My tailgate now hangs on the wall of my garage. You might have to do the same if your tailgate is bad.

Yup.. That's what it looks like. the brackets are way bigger on my truck, and have square holes to fit the round rod lol. I have an old picture when I had the tailgate on.
