One day it was suddenly hard to start. Then one day it wouldn't start. Burned factory starter and put in new. Now it starts better, only due to cranking speed. Usually sputters at start up.
Runs equally bad on both tanks. Jerks bad at low rpm, missing with or whithout a load. Above 2krpm all is well.
What I do know-
No codes
Timing does advance with rpm and vacuum.
40psi fuel pressure.
new-Map tested 170hz@6,500ft elevation.
4month old tps tested
ECT(the one near the thermostat, right?)seams fine.
I measured 4.6v at the ECT connector. Should it be a solid 5V or is this normal?
egr functions when vacuum is applied.
I unplugged the iac thinking maybe it was sticking open, motor died right away. I was able to feather throttle to idle speeds, then rev it up and same hesitation was there.
I'm out of ideas and I need to tow my trailer up a BIG hill in the next few days. Please help or I will have to use the wifes stupid Xterra to tow Oh the shame of it.

it sounds like you have a vacuum leak. your symptoms are indicative of such, and typically the higher the rpm/load, the less prevalent the symptoms are.
id start checking for large vac leaks; egr, brake booster, vacuum canister, etc.
the ole can of carb cleaner/brakeleen/ether will highlight any leaks in the form of higher rpm while spraying.
I'm thinking PCM failing or has failed.
Reason for that...
OP stated that he received System Pass code on KOER. He should not have got that considering how poor the engine is running. At the most, a system error should have been read with irregular idle, unable to pass at maintaining idle for "X" variable.
