Pull plugs and inspect. You said that you waited some time to replace/repair the O2 sensor. Chance's are the plugs could be fouling since you inadvertently had the truck running in closed loop the entire time. Really, it wouldn't hurt to replace the plugs again. With it constantly in closed loop, the PCM ordered for wider pulse from the injectors, thus creating wasted, unburnt fuel to be expelled through the exhaust. Lots of fuel didn't get ignited as it should have been.
Need to start looking at the basics again. And yes, I know plugs cost money, but when they foul out, they create other problems.

Good suggestion. I believe you have your open and closed loops mixed up my friend...
Picture looking through a small section of exhaust pipe that has an o2 sensor installed. The circle is "closed", and the o2 sensor is in its bung, reading exhaust. Take the o2 out, the circle is now open, as there is no o2 sensor filling the bung, it's "open", and not reading o2.
I'm sure you know what's what, but they're easy as hell to confuse. . .bet ya won't confuse it with that visual!
I used platinum motorcraft spark plugs in it probably should replace them and get back to you guys but when the o2 sensor was bad I didn't think it ran to bad better then it does now.
